Botulinum Therapy (BOTOX®)  FAQ

 

What is Botox®?

Botox is a purified protein produced by Clostridium Botulinum bacteria that has been used in medical practice for many years to treat muscle spasms.  It can be used safely for cosmetic purposes to improve facial appearance by temporarily reducing facial lines and wrinkles.   During treatment, minute doses of Botox are administered via tiny injections directly into the muscles that cause the wrinkles.  Each treatment can normally be completed in just a few minutes and recovery is almost immediate. 

“Botox®” and Vistabel are the registered trade names of the purified Botulinum Toxin manufactured by the company "Allergan".   Other effective Botulinum toxins are available, including “Dysport” manufactured by the company "Ipsen".

 

 Where is Botox injected?

Botox works best on wrinkles that are caused by active contraction of facial muscles.  There are three main areas commonly treated.  1. Frown lines between the eyebrows. 2. Forehead wrinkles.  3. “Crows feet” lines at the side of the face.

 

Other uses of Botox include the treatment of excess perspiration from the axilla (armpit), hand and feet.  It can also be used to treat facial spasms (tics) and squint.

 

How does Botox work?

It works by blocking the transmission of nerve impulses to the injected muscles.  This reduces the activity of the muscles that cause persistent lines to form.

 

When will I see results from a Botox treatment?

Response is usually seen 3-7 days after injection.   So within days of receiving Botox injections, you should see a marked improvement in facial lines.  Lines can continue to improve for up to a month.

 

How long does Botox last?

The effect of Botox is temporary.  The beneficial result from treatment with Botox typically lasts for 3 - 6 months.   At this point a repeat treatment will relax the muscle and reduce the lines again.  If you discontinue Botox treatment the wrinkles will gradually return and you will simply look like you would have done without any treatment.

 

Who should administer Botulinum Therapy?

A detailed knowledge of facial anatomy and accredited training in injecting Botox is essential.  Qualified therapists and facial surgeons who regularly treat facial pathology are well placed to offer this treatment.

Since June 2016 the General Medical Council have issued regulations regarding the cosmetic industry in The UK.  Botulinum Toxin must be offered only after a face to face consultation with a qualified prescriber.

 

Who should not have Botox?

Botulinum therapy should not be used if the patient is pregnant or breast feeding.   Botox should not be used in the presence of infection at the proposed injection site.  It  cannot be used in individuals with known hypersensitivity to this treatment.   Patients with neurological disorders such as Myasthenia Gravis or Lambert-Eaton syndrome may be at increased risk of serious side effects.  It should not be used in patients with any condition affecting the ability to swallow.

 

Are there any side effects or complications?

Side effects after Botox therapy are uncommon but possible effects include mild headache, bruising, swelling, redness, tenderness and pain during injection.   These symptoms will resolve within hours or a few days.  Other rare complications include asymmetry (uneven effect), temporary drooping of eyelids or eyebrows.  This is temporary but could last several weeks. Bruising may occur after Botox injections.  Substances that increase this risk include, aspirin, Vitamin E, non-steroidal anti-inflammatory drugs and warfarin.  These should be avoided for at least 7 days before injection.

 

There may be a higher possibility of side effects if you do not follow specific post-treatment instructions.  In particular you should avoid bending over, rubbing or massaging the treated area for 24 hours. 


Will my facial expression look natural?

Although the cosmetic results are visible, a treatment with Botox will not radically change your facial appearance and should not  make you look un-natural.   The muscle activity that causes frown lines is simply reduced, so you can still frown or look surprised to an appropriate degree.

 

How much does it cost?

Botulinum therapy is sometimes charged according to the amount of toxin used and number of facial areas treated.  There are 3  areas commonly treated

Dermal Fillers - Frequently asked questions - FAQ

 

What is a Dermal Filler?

Dermal fillers are substances commonly injected under the surface of the skin to fill out deeper, static facial lines and wrinkles. They can also be used to restore soft tissue in the face and elsewhere in the body that has naturally declined as one grows older. 


There are many different types of Dermal filler available in the UK.   Some are already tried and tested over many years and others are newly available but may prove safe and effective in the future.  The most widely used filler substances are based on Hyaluronic acid.  We commonly use Juvederm (made by Allergan) and  Restylane® (made by Q-Med).   They have been used for many years throughout the world with an extremely high safety record.  There are different preparations of Restylane  and Juvederm with varying qualities, designed to be injected in various parts of the face and body for optimum cosmetic results.


Hyaluronic acid  occurs naturally in youthful skin.  It helps the skin retain the natural soft,  supple quality of youth.  As we age the amount and quality of Hyaluronic acid gradually declines, leading to the usual loss of facial fullness and development of deeper wrinkles.


How are cosmetic dermal fillers made?

The Hyaluronic acid in Restylane and Juvederm is biotechnologically produced by bacterial fermentation.  It is then extracted, purified and stabilized to increase its longevity in the tissue.  It is prepared in a gel which is then administered in syringes as required.


Where are Dermal fillers used?

Different types of filler are used in different parts of the facial skin.  For example, standard Restylane is very versatile and can be used in most areas of the face with good results.  Restylane Touch is designed to be injected very superficially in the skin to treat fine wrinkles.  Restylane Perlane is designed to be injected rather deeper in the skin to fill out deep facial lines and wrinkles.


Dermal fillers are more suitable than muscle relaxing injections for the treatment of static facial lines.  ie. If a wrinkle is still visible when the face is relaxed then Botox may not work and a dermal filler is required.   Dermal fillers are particularly effective in treating the deep furrows that extend from the nose to the corner of the mouth, and from the corner of the mouth to the chin (marionettes lines).  Deep static frown lines between the eyebrows are also effectively corrected with fillers.


When will I see results from Dermal filler treatment?

You will see the result immediately.   The facial line should be much reduced straight away.  There may be some temporary redness and mild swelling which will settle in a few days to give the final effect.


How long does it last?

The effect of most dermal fillers is temporary.  The beneficial result typically lasts for 6– 10 months.   At this point a repeat treatment will improve the appearance again.  If you discontinue filler treatment the filler is gradually absorbed naturally and the lines will slowly return.   You will simply look like you would have done without any treatment.


Any side effects?

Some common injection-related reactions might occur.  These include redness and swelling, pain, itching, discolouration and tenderness at the injection site.  The effects are generally mild and last for a few hours or days only.   Hypersensitivity or allergic reactions to Hyaluronic products such as Restylane are extremely rare.   Bruising can occur, particularly if you are taking drugs that reduce clotting. eg aspirin or warfarin.
